How to clean out the required pending action item

We have a requirement to clean up the pending action items labeled "Participant Requires Primary Care Physician." I attempted to clear these by running Manage Action Items for All Employees, but they are not being removed.

Please suggest a solution to clear these pending action items and advise how we can prevent them from being triggered in the future.
