REPORT: Filter on response to one question, and include answers to other questions

Example: I have a transactional survey with 10 questions.

I want to create a report that shows only the responses where question 5 = Yes

I want the output to show the answer to question 5, question 2, and question 1 as well as the contact/incident information

In order to build a filter, I must first limit the results to the survey, then to question 5, then to that result.

Now I can't add back in the responses to questions 2 and 1 for that contact/incident/question_session

The closest thing I have found is the Survey Question Correlation Analysis. 

This report essentially filters the table to question 5 responses, and then filters uses the same table with an alias to filter to question 2 responses.
