Report showing Agent who add Incident Comments

I am looking to create a report that will show the name of the agent that added comments to an incident. We use a Custom Object to hold the comments that an agent adds. Currently the report has the Incident Created by field that shows the agent. However, when another agent or supervisor adds a comment to that incident it still shows the original agent. If I change it to use the user transaction it shows all the agents on every comment resulting in multiple identical comments with different agents. I have also tried using the Threads table and it
