Combined Trimester and Annual Performance Forms

We have both annual and trimester performance evaluations.

The annual and third trimester forms overlap in timing and to ensure a good end user experience would like to combine this in a single document/form. We need to be able to rate competencies, goals and provide an overall summary that feeds the trimester and annual ratings in order to feed merit and bonus respectively.
