Why are approved certification pending actions carrying forward into

Summary:

The client completed a full audit on all dependents this summer under the HCC Dependent Verification life event seen in first screenshot below. Dependents had certifications uploaded and were approved along with a received date. When the client processes a sequential life event, the pending action is triggering again, the certification attachment remains, but the pending action requires a received date and the approval to be checked again. Why is this happening if the dependent has already been certified under a prior life event? I would expect only a newly added dependent to have a pending action triggering. Please advise.
