Wrong PDF generating for confirmation statement

When users try to use the Print All Benefits option the wrong PDF is generating. There are extra columns that are not on any of our templates.

In a development environment we have removed all of the custom templates and only have the Out of the box version available and it is still generating the wrong PDF. We are not sure how to get the correct version to generate.

There was a fix for this in the 25C patch but a CWB we took shortly after the patch wiped the fix and the problem is back now.
