HR unable to save evaluations in Performance Documents for some users

Summary:

As a HR user we are experiencing issues when saving evaluations in a Performance Document on behalf of the employee or manager. This is only for a few cases and the error message Not Found does not give much indication of what the error may be. Has anyone else experienced this and have any guidance?
