Client has 3 questions that have to be answered T/F before enrolling into the HSA

Summary:
Has anyone ever encourterd a client that enforces their employees to answer 3 questions before enrolling into their HSA? If so, any ideas on how to configure this?
My only thought was to add it to the acknowledgement.

Here are the T/F Questions:
- I am NOT currently covered by any medical plan that is not a high
deductible/consumer directed medical plan (note - a high deductible/consumer
directed medical plan has a deductible set by Federal law at a minimum of $1,650 for
an individual and $3,300 for a family plan). - I am not enrolled in Medicare Part A and/or Part B or Tricare.
