Why would employee show up multiple times in benefits activity center

When searching for an employee in the Benefits Activity Center, employees are showing more than once for the same assignment, however, the employee does not have multiple rows with the same assignment number. The employee should only be shown once in the Benefits Activity Center. Has anyone else come across this issue. If so, what was the resolution?
