Issue with item structure updates fail to consistently relink to work definitions

We’ve been facing an ongoing issue where processing item structures to work definitions does not behave consistently. The expectation is that updates from item structures should properly relink to work definitions, but this only works about half the time. In some cases, the link breaks completely and the work definition is not attached. If we manually add it back, the system sometimes re-adds it later during processing, causing duplicates and failures. Does anyone have a similar experience or know how to address this?
