unable to see some actions in the talent review meeting

I have created a talent review template and enabled all of the following actions:


When I create and conduct the meeting, only the following actions are visible:

I am unable to see the actions to add notes, add tasks or add development goals. I am also using the seeded Human Resource Specialist role.

Has anyone previously faced this issue?
