When I create a record on the Account or Contact object and save it, the record is saved successfully, but it does not appear after saving. What could be the reason for this behavior?

I am working on Account and Contact objects where users can create new records. The record is saved without any error messages, but once the save is completed, the record is not displayed in the application. This issue occurs consistently for both Account and Contact objects.
There are no validation errors shown, and the save transaction appears to complete successfully.
