Form Submitted with no contact creation

Hello! :)

Eloqua form was submitted on Jan 21, 2026 & we see the form submission only after importing the excel file of the form submits.

The contact was not created - that is expected looking at the form processing steps. I have tried to reproduce this behaviour in Sandbox but I failed.
Do you know what might be the reason behind this case? We see the form processing steps have and (ideally had it previously as well) - the step for updating the contacts.
