Expert Advice Request: Prerequisite Form before creating a job requisition

Summary:

Hi Team,

We currently blocked our Hiring managers from creating the requisition in ORC. We want them to fill a form (similar to request new position etc.) mentioning is that a replacement, new opening etc. , productivity details etc. and that form should go to approval to VP. Once form is approved, recruiter should be notified and then recruiter will create a requisition.

What's the best way to acheive this in Oracle?
