Hi Everyone,

I am encountering the following error when submitting a form with attachments:

OPA-ERR-001: Error setting attachments in project - Upload was not confirmed by file service

The attachment uploads successfully in the UI, but upon submission, the process fails with the above error as it shows in the message logs.

Has anyone encountered this issue before? If so, could you please share the root cause and any troubleshooting steps or resolutions that worked for you?

Additional details:

  • Attachments appear to upload successfully before submission.
  • The error occurs during the submit process through a knowledge article to create a Help Desk Ticket.
