Certification based Rates for Medical Plan
Case:
Full-time employees who have been employed a year or more and submit their annual wellness form by December 1 of the current year are eligible to receive a $50 medical premium credit each pay period. All employees receive the premium credit the first year of employment.
Employees who do not submit their wellness form by the deadline are not eligible for the premium credit. As a result, their medical premium will be $50 higher per pay period than employees who have submitted their wellness form.
Example:
For example, an employee who elects Employee Only medical coverage is assigned Option Code 1 if they qualify for the wellness premium credit. If the employee does not submit the required wellness form by the deadline, their enrollment is changed to Option Code 5
