RestAction Error message when trying to create a new version of questionnaire

in Human Capital Management

Issue: When I try to create a new version of the questionnaire, I receive the error message "Error response during RestAction"

Background: I created questions, added to a questionnaire template, added the template to the questionnaire then the questionnaire to a Journey template. This questionnaire is "in use" as the Journey template is currently being assigned and completed.

I made a change to a question "instruction" by creating a new question version. The questionnaire does not reflect the updated question. I need the new version of the question to appear when a new questionnaire Journey is assigned.

How do I accomplish getting the new version of the question instruction into the Journey?
