Comments on Questionnaire are not saving after 26C

in Performance Management

After 26C, our Questionnaire section within our Performance Document is not saving the comments which are all required. We have 3 questions listed and when the manager attempts to enter the comments and select Save and Close, the form seems to save but when the manager clicks "SUMBIT", an error displays. Our section is named "Development Plan" and it's a questionnaire section.

I've tried to create a new version of the Questionnaire and removed the required flag and I can see the difference on the performance document. However, only 2 of the 3 questions are working properly. This is odd behavior and the only change to this Performance template was the recent 26C patch. The Performance cycle has been open for weeks prior to the patch loading with no issues. Has anyone else encountered this issue?
