Save/Update button not saving/updating the details entered in Check-In Questionnaire Template

We appear to have an intermittent issue where a manager's comments entered in Check-In Questionnaire text boxes and clicks on 'Mark as done' its not saving the details.

The user has selected "Mark as Done" and clicked "Update", but the change was not saved and the status was not updated.
