1. Did it ever work fine, in this installation?

2. Could you try detaching and reattaching the library to the form and then recompile the form?

3. Try recompiling the form and library in command line, not Builder:

frmcmp_batch.sh userid=<username>/<password>@<DBalias> batch=yes module=<module> module_type=<module_type> compile_all=yes

4. Make sure there is a single version of the plx on the server, in FORMS_PATH

5. Make sure patch:26996652 is applied in your Forms Home

at last, after recompiling both PLL and form, it worked.
