Please verify below setups.
1 ) Navigate to Action types and set the below for Action Type'approval-group chain of authority':
Ordering Mode =Parallel and Voting Method =First Responder Wins and click on Apply
2 ) Navigate to approver groups and create a approver group with Voting Method as 'First Responder Wins'
3 ) Navigate to Rules->Click on Create
i)Give the name and rule type as 'List Creation'
ii)Click on Next and Add condition created in step 4 with condition type as 'Ordinary'
iii)Click on Next and Take the action type as 'approval-group chain of authority' and select the relevant Action
iv)Click on Next and review the data and Click on Finish

Re: Why multiple Infrastructure patches QDFSP?
Hello,
This is because there are 2 majors version in Jul2025 : 24.1.x & 25.1.x.
Usually, The QDFSP for a given month includes binaries for all majors versions.
You need to select only the binaries related to your target version.
Personally, I never download QDFSP (just to save my disk space). I download only some specific zip of my target KVM version, e.g. for : 25.1.9, I download only these binaries from KB153930" target="_blank" rel="nofollow noopener ugc">KB153930 (888828.1 ) :
Patch 38376204 - Storage server software (25.1.9.0.0.250908)
Patch 38279796 - Admin/RDMA network switch (10.3(7)) and InfiniBand network switch (2.2.17-3) software

What is the output of this query: select max(maxquerylen) from v$undostat;

I think there is a problem for several reasons:
- Since the Nov 13 the undo size increases indefinitely only on node 2
- There are no transactions which are uncomminted
- I restore the database from RMAN backup on an other test environment with no third application started and the extents are still active
- From the restored test environment, I run insert tests and when the transaction use the segment '_SYSSMU108_35568451$' the extents created for this keep active status after commit or rollback statement.

Thanks for trying to help here, actually your inputs helped us creating the servlet filter for the identification of the redirection and make it work as per our requirement.
It was went live by the end of Oct 2025 and working fine since then.
