approval process not working

Dear All

We are using PSFSCM 9.2 image 0 PT 8.53, we are using approval process on requisiton stage based on origin. Due to a need to increase approvers and as it couldn't be increased plus + sign not available I've cloned the existing one review below screenshot make it inactive and unticked the default tick:

Thus, in stage/test environment I've cloned it review below the new clone. I've checked almost everything by now it's working fine, by changing an old requestor (say Mr. A) origin and make a new requisition from my ID the system send it correctly to the new approver also the old one's remained approved with the previous approver name (Kindly guide me if anything also needs to be checked).
