Unable to enroll

Employee - was unable to enroll in benefits so he asked if I could enroll him in manually. I went in like I usually do to Ben Admin, and I keep getting error messages when I try to enroll employee in a benefits plan. I tried the EPO and got one message, then I tried the PPO plans and got another message.

employee wants to be enrolled in the Limited PPO plan but I am unable to enroll him. I had HR look at Person Management/CoreHR and they said things look correct on their end.

Please assist. (screenshots of error messages in Oracle are attached).
