Certification Restriction Formulas at the Program or Plan Type?

I see there are some Benefit Certification Restriction Formulas configured at the Plan level:

However, I do not see any at the Program or Plan Type level. Do these have to be created or are the delivered somehow/somewhere else? We are currently implementing Cloud for a first time client and do not want benefits requesting certifications for workers enrolled in the legacy system.

Also, if both Vision, Dental and Medical require the same certification. How do you limit so the system only asks for it once?
