Display range corresponding to the selected performance rating in Performance Evaluation

Summary: Our client has 5 performance rating levels. The first score range is from 0 to 4, the second is from 5 to 24, the third from 25 to 50, the fourth from 51 to 75 and the fifth, from 76 to 100.

At the time of the evaluation, the evaluator must select one of the rating levels, and within that level, also has the option to select in a slider, one of the steps that corresponds to the range of points above, but without being able to visualize the corresponding number. Does anyone know the best way we can do this?
