Offer Letter Approval not working for a specific department

Summary:

We have defined a two level approval process where first approval is required from the Recruiter and the second from the Deputy HR Commissioner. It is working fine for all the positions which are selected at the Offer Letter Creation phase except one which belongs to a certain department. For the certain department, the offer is ready to be extended post first level approval itself and the second level approval is not even showing up in the Transaction Console or the Approval History.

What may be the potential cause of this ?

I tried creating the Offer for the same candidate by changing the position to two different department and it is working perfectly as designed for one but not for the other one.
