Possibility of having 3 forms in the same Performance document

In a performance document, we would like to have 3 forms (annual interview - professional interview - 6-year review (a French interview)), so there are 3 different questionnaires in the questionnaire section. My questions are as follows:
Can we have several questionnaire sections in a single performance document and how we can have 3 forms in the same performance document as we are limited by the number of sections and can only have one questionnaire section in the performance document ?
Can we divide the questionnaire section into 3 parts? And place these parts in the order you want in the performance document, e.g.: part 1 of the questionnaire concerning the annual interview placed before the skills section, part 2 of the questionnaire concerning the professional interview placed after the Goals section?
