How do I connect a questionnaire to a general feedback template in Redwood design?

Summary:

I am creating a new feedback template and more specifically creating a general feedback template. I do not have a questionnaire that is connected to this template type. I was able to create the questionnaire for the performance goal feedback in the questionnaire template section with the Performance 360 subscriber. What subscriber would I use to create a questionnaire that would appear in the dropdown for the a general feedback template type?
