+ sign under goal plans not working while adding goal

Hello,

I have created a library goal under performance goal library , however when i now go to goal plans and click on '+' i am unable to add any goals or details.

Ideally the new goal page should open up as below, but nothing populates when i click on add option. Can someone help me resolve this ?
