Check-Ins content is missing not displaying on scheduling on the invite

Hi all, reaching out for some help please around Performance Check-ins in Redwood.

Create a Check-In Template for a POC, associated a no response questionnaire to it as they are just prompts for discussion for the manager and employee, however when scheduling one or viewing a scheduled one, there is not questionnaire to see/view?

I do however see the goals and feedback.

Also any ideas what general discussion topics does actually shows/displays - could do with using that for the manager and employee discussion prompts but I can' seem to add anything?

I also used the comments to display this with an external link but that does not work either!
