Why do we see old goal plans name in the notification when employees complete their goal?

The notification uses the token ${GoalPlanName}, but instead of pulling the current active goal plan, it is displaying an old goal plan name that ended in 2022. That goal plan has since been marked inactive, yet it still appears in the notification.
