How to papulate grade levels based on competencies in performance document

Dear Experts,

We were created competencies in the form of questionnaire with list of values as shown below.

For Ex: Employee have Grade = MG1A then Level 1: Awareness Should auto papulate at Questionnaire level in performance document.

The Below Question is Competency:- Presentation Skills

Can you please suggest any workaround , that would be great help to us.
