Employee Received Feedback FYI Notification Sending to Previous Manager

Has anyone noticed feedback notifications delivering to an employee's previous manager?

It was recently brought to our attention that when an employee received Feedback, the previous manager was the person to receive the FYI notification as opposed to the employee's current manager. If anyone else has had this happen, how did you solve and how did you ensure to prevent it from happening again?
