Comment Box Missing in All-in-One Evaluation

Has anyone else experienced the comment box in the All in One Performance Evaluation not displaying when attempting to add comments?

Our performance documents are properly configured to allow comments, and when a manager navigates directly to a performance document they are able to add comments. However, if they attempt to evaluate from the All in One space, once they click the icon to add comments the panel drawer never loads the actual comment box. (Or any of our other additional information that is configured to display.)

Has anyone else experienced this? How did you solve?
