Check-in comments not saving

We appear to have an intermittent issue where a manager's comments entered in Check-In Questionnaire text boxes and Goal text fields are not saving, even though the manager is clicking the Save and/or Update buttons.

The manager acknowledged they kept switching between the Questionnaire and the Goals notes, but after saving the Check-In they found that none of the notes entered had been saved.
