Best Practice : performance questionnaire

Hello,
In our performance document, we have a questionnaire with only free text fields to assess job performance (description of tasks and examples of concrete achievements).
Each year, employees and managers are asked to complete this questionnaire, which takes time.
Job performance is one of our key performance objectives. We, therefore, have a goals section which is also assessed using a rating scale.
We have had some negative feedback on the user experience of this questionnaire :
- missions and examples have to be re-entered every year
- it takes a long time to complete for each employee
So we're looking for ways to improve the experience.
