Cannot set up new "Payroll Item"

When I go to Lists > Employees > Payroll Items > New, I get a form that I can fill in only the following:

Item Type
Item Name
Pay Code

Then the rest of the form is blank, I know there should be a field called "Expense Account", but it doesn't show up.  I try and enter the above 3 fields and hit save but it asks me "Please enter a value for account", but there is no place to enter the account.

When I first go to this page it flashes with all the information for about half of a second then the page goes white.  I have tried on Firefox and Safari.  I have a Mac.  I will try on my PC when I get to work.
