Alerts Composer - Document Approved or Rejected - Benefits

I enabled the alert for the Document Approved or Rejected in Benefits. I processed a life event, had a certification approval, uploaded the document as the employee, then approved it as an admin, and the employee did not receive an e-mail. Is there something I am missing? Does this work only for Proof of Good Health Documents? I am trying to use it for marriage and birth certificate.
