Linking Online Case form submissions to existing customer records

I have created a few Online Case forms. They are actually for people to submit questions or provide feedback after coaching sessions. They are not actual 'cases/incidents'.
I want to be able to send links to these by email to certain customers.
The problem that I have is that when the customer follows the link and submits the form, Netsuite is creating a new customer record and assigning the case to it.
I have tried passing the forname, lastname and email address as parameters in the link to the form- whilst they appear automatically on the form, it still creates a new customer record, with the added disadvantage that the newly crested record is unsubscribed - which then makes a merge impossible.
