Blind Shipping - How do we specify the Packing slip form on a per-fulfillment basis?

We have two Item Fulfillment forms at "Setup -> Customization > Transaction Forms".

They are...
* Regular Item Fulfillment {-Preferred
* Blind Item Fulfillment

We also have two Packing Slip forms at "Setup -> Customization > Transaction Forms".

They are...
* Regular Packing slip {-Preferred
* Blind Packing slip

The "Regular Packing slip" form is set as the preferred Packing slip form.  The Blind Packing slip form uses a PDF layout that doesn't include our logo or company name.

We want to be able to switch certain fulfillment's over to use the "Blind" packing slip form.  Switching the form used on the fulfillment should then make it print the "Blind Packing Slip PDF" when that fulfillment' has it's print button clicked.  Is that possible?  Why is there no field in the "item Fulfillment Form Set-up" that says "Select Packing slip to use with this fulfillment form".
