Help! Cannot Add Related Items to Existing Items

Since yesterday, I have been unable to add related items to any of our existing items. Nothing has changed with the item entry form, it just seems to have broken.

Here's my workflow, can anyone reproduce this?[LIST=1]
[*]Navigate to any inventory item record and hit edit.
[*]Add at least one related item to the related items tab.
[*]Save form.
[*]Receive this error:

There is already an item with that name.
. Go back, change the name and resubmit.[/LIST]This error occurs on standard inventory items and on matrix items alike. It only happens when I add related items, so far I have not found any other fields that are affected.
