Unauthorised code release ? External Sales Order

After a lot of testing, I finally found the problem as to why my external sales order form started failing all of a sudden.

If you untick a tab that has unselectable mandatory fields, the external sales order form will not submit, instead giving you an "enexpected error".
Our current form worked for a long time before something changed in the software - not from our end.

Someone might be releasing code changes outside this structured methodology and breaking our systems.

Does anyone else have this problem ?
