Opportunity Last Activity Report (again)

There was a thread from about a year ago regarding how to do an opportunity search that showed only the last activity done on each opportunity.

I have been over that thread a dozen times and have done exactly as it suggested, even using the screen shots.

No matter what I do, I get "ERROR: Field Not Found" in the Formula Fields

I would be ever so grateful if somebody could help me understand what I'm doing wrong and get it working.
