Default Color Scheme Turning White?

This question doesn't really fit anywhere, but I'll start here.

I've had a few instances now where the "Default" color scheme that users have turns from blue to white.  This is a problem because it makes some menus and buttons invisible, even though they do appear to actually be there.  Users are understandably freaked out by this as they can't see or get to mission-critical areas on screens.

Switching to any other color scheme and back fixes the issue.  I haven't filed a case because I can't make this happen on command and I don't want to make someone wait while they can't really do anything.  Is this a known defect?  Thanks.
