Issue 117362: Case Notifications > Online Case Form > If Add Unsubscribe Link to Message Footer = T on the Auto Reply E-mail Template the Subscribed as E-mail that shows is for a different Customer Details

Check your online case forms; if you have an unsubscribe link in them you may have trouble.
