Basic Email Subscription Form Help

Hi everyone,

I know this sounds very basic, but I am new to the whole "creating online customer forms" thing. All I am trying to create it a simple online form  within Netsuite that allows customers to subscribe/opt-in to one or all of our three email newsletter campaigns, through the use of check-boxes. Why does this seem to difficult? Thanks in advance!
