Item Name Appearing Twice

On transactions, all of our line items show up with the item name twice, separated by a space. So for example 'ITEM001' shows like this:

ITEM001 ITEM001

They appear like this when editing the transaction (adding items), and when in view mode as well. It's very annoying and superfluous, any help getting rid of this appreciated!
