CSV import of new demand plan

Trying to add a demand plan for an item that previously has not had one. One world account with Subsidiary and location. I can update a demand plan using the demand plan internal ID. Have tried every combination for Subsidiary and Location.  Hard coded names, column headers and data, internal ID's and combinations of these. They all generate the same error message:

Invalid location reference key xxxxxx  for subsidiary <NULL>"

Any thoughts?
