Customizing Form (Help Please)

Hey friends,

HELP!  We need to make a form live ASAP and have only a couple small things to figure out:

I have some questions regarding customizing online forms.  This should be an easy one as I know these questions have been asked many times (yet they are all answered in different ways on this forum).  However, I'm not Java-savvy and would benefit a whole lot from some quick pointers.

Right now I'm using a default NS form which is fine for now.
I need do the following:
• Have all customers who submit the form be automatically assigned to a single customer category.
