Checkout url changing?

On two different occasions we have had our website checkout url change from our custom domain secure url back to the default netsuite url.  The first time we caulked it up to human error (though we doubted it) but now that it has happened a second time, we believe there is something that is changing it back.  Any ideas what could be causing this switch?  There are no system notes for the website setup which is a complete shame. 

First time it mysteriously changed was 10/8, found and fixed 10/17.
Second time it mysteriously changed was 10/21 and found and fixed 10/25
